For injured workers
Before your first visit, your claim needs authorization for treatment at our office. If you’re not sure whether you’re authorized, call us with your claim number and your adjuster’s contact information, and we’ll help you check.
Work injuries we commonly treat:

- Neck and upper back pain from repetitive work
- Herniated or bulging discs and sciatica

For adjusters, nurse case managers and networks
We accept authorized referrals and work with networks including MedRisk. When we receive a referral, we:
- Schedule the injured worker promptly
- Submit requests for authorization on the DWC Form RFA
- Send progress reports with objective measures such as pain scores, range of motion and functional goals
- Keep you updated on work status and treatment progress

Do I pay anything for workers’ comp treatment?
Authorized treatment for an accepted claim is billed to the claims administrator. Call us before your visit so we can confirm your authorization.
Can I choose CRM Wellness Center for my care?
It depends on your employer’s medical provider network and your claim. Ask your adjuster, or call us and we’ll help you find out.
How many visits will I get?
Visits are authorized by the claims administrator. California limits chiropractic visits per injury, with some exceptions. We document your progress so the right amount of care can be requested.
